Case Manager/Family Support Worker – Gardner
CSO is looking for a Case Manager to join our Family Support Services team in Gardner. Starting salary: $38,000-$44,000/year.
Responsibilities- Provide skills training and assistance in daily living activities including basic living, interactive and parent skills. Assist in developing communication and conflict resolution skills and act as a role model for interpersonal relationships.
- Help parents meet children’s daily needs; observe and document child growth and development.
- Assist with family advocacy needs; facilitate “mediation” as appropriate.
- Provide counseling and intensive support to consumers during psychiatric crises or periods of transition.
- Assist consumers with identifying appropriate mental health and medical services and accessing services of choice.
- Provide assistance with developing and maintaining a natural support network of family and friends.
- Provide transportation for consumers as appropriate, following program guidelines.
- Maintain appropriate contacts with family members, others in the natural helping network, outpatient therapists and others as necessary to support consumers.
- Bachelor’s degree in a mental health field.
- Experience working with children and families.
- Valid driver’s license and reliable transportation.
