** Position Overview
** We are seeking an experienced
** Home Health Marketing Liaison
** with a strong background in the
** Los Angeles healthcare market
** and proven success in cardiology-related referral development. This is an excellent opportunity for a healthcare sales professional passionate about building relationships and driving growth in home health services.
** Key Responsibilities
*** Develop and maintain referral relationships with cardiologists, hospitals, case managers, discharge planners, and physician offices across the LA market
* Promote home health care services, emphasizing cardiac care and chronic disease management programs
* Consistently achieve monthly and quarterly admissions and referral targets
* Educate referral sources on clinical outcomes, patient benefits, and regulatory compliance
* Monitor market trends, competitor activity, and payer mix
* Collaborate with clinical and operations teams for seamless patient transitions
* Document all sales activities and referral data in CRM systems
** Qualifications
* ** 3–5 years of home health or healthcare marketing experience in the Los Angeles market (required)
* Proven success working with cardiology practices or cardiac-related services (strongly preferred)
* Established network of referral sources in LA and surrounding areas
* Bachelor’s degree preferred (Healthcare or Business-related field a plus)
* Excellent communication, presentation, and negotiation skills
* Self-motivated, organized, and results-driven
* Compassionate, patient-first mindset
* Valid driver’s license and reliable transportation
