LPN Licensed Practical Nurse

Hybrid position; in-person required twice monthly in Liberal & Garden City, KS. We re looking for a Licensed Practical Nurse to join our team. Mosaic relies on the nursing team s ability to assess and monitor all aspects of an individual s care, including medical and psychiatric concerns.

• Collaborate with community healthcare service providers, nutrition services, and behavior specialists to provide individuals served with quality care.
• Coordinate and oversee all healthcare appointments including appropriate follow-ups.
• Educate and enable the Direct Support Team to administer medications and perform other tasks according to Mosaic policies and state licensing requirements.
• Maintain and review medical records of individuals served to monitor pertinent concerns and provide documentation detailing the delegation of healthcare procedures and nursing tasks.

Monday–Friday part-time day hours; flexible schedule.

• Associates degree required.
• Licensed Practical Nursing/Vocational Nursing certification in good standing for the relevant state location of the job.
• Minimum of one year of LPN experience or 3 years of CNA/CMA required.
• Certified in CPR & First Aid.
• Valid driver s license if required to transport individuals.
• Work requires frequent physical activity including extended periods of standing, walking, and bending with occasional periods of sitting, kneeling, climbing, stooping, crouching, squatting and balancing.
• Work also requires constant reaching between knee and shoulder level as well as frequent reaching below knee level and overhead.
• Work requires occasional independent lifting up to 25 pounds, frequent push/pull up to 40 pounds of force and the ability to safely transfer 50 pounds.
