Responsibilities- Working with hospital case managers, the patient, and the patient’s family to provide placement for skilled nursing services or skilled rehabilitation therapy.
- Communicating with the appropriate interdisciplinary team members (IDT) to ensure a smooth transition from a home or healthcare setting to a Human Good Skilled Nursing Health Center.
- Communicating the patient’s plan of care to IDT to ensure consistent quality care.
- Monitoring reviews and managing the inquiry process through the Referral Manager System as needed to assist the admissions team.
- Supporting the admissions counselor with inquiries from families, hospitals, assisted living, and other referral sources.
- Maintaining communication and rapport with hospital accounts, accountable care organizations (ACOs), and other referral sources.
- Ensuring utilization of standard admission process via HER system; manages inbound continuity of Care Documents (CCD) via Care quality HER integration.
- Communicating the new admission’s specific needs to IDT, primary care physician, or other providers to ensure Person-Centered quality care is provided and to support discharge goals.
- Providing other related nursing duties and responsibilities as needed.
- Current RN license in the state for which applying.
- Current CPR certification.
- Prior nursing experience in senior care, skilled nursing, post‑acute or sub‑acute care is preferred.
