Job SummaryWork with the Chronic Diseases Prevention and Self‑Management Program team to enroll or lead program classes.
Job Description Program Recruitment (50%)- Recruit program participants, adhering to each program's eligibility criteria.
- Participate in recruitment events, pitching the program information to potential participants.
- Participate in investigator meetings and workshops.
- Adheres to internal controls and reporting structure.
- Conduct Chronic Diseases Prevention and Self‑Management Program classes, as assigned.
- Collect and report point‑of‑care blood work, biometrics, and surveys from program participants.
- Record and report participant data and outcomes.
- Performs related duties as required.
- To be trained as a lifestyle coach or leader within 3 months of hire, if leading any applicable classes.
- Earning a degree in public health, healthcare administration or related discipline.
- Bilingual (English - Spanish).
- Experience in implementing lifestyle modification programs.
- Experience working in academic health centers.
- Experience in working with participants of all ages.
Commensurate with experience.
