We are 21st Century Home Health Services (21

HHS), the largest home‑health agency in San Francisco and quickly growing throughout the Bay Area. Our clinicians provide skilled services in patients’ homes to help them live safely and productively. All opportunities at 21

HHS require field visits to patients’ homes.

Base pay range: $/yr – $/yr. Additional incentive bonus up to $20,000 depending on level and performance.

Anticipated Territory: Galt Area.

Responsibilities:

• Perform home evaluations and develop individualized care plans.
• Work with patients to improve mobility, movement, and independence.
• Create detailed treatment and care plans for each patient.
• Communicate and coordinate with physicians to ensure high‑quality care.
• Evaluate and report patients’ progress to MD.
• Supervise Physical Therapy Assistants (PTAs) as allowed by agency standards.

Qualifications:

• Current unrestricted California Physical Therapist license.
• Current BLS/CPR certification.
• Valid California driver’s license.
• Knowledge of local community resources.
• Understanding of individual development and human behavior related to illness/injury, and cultural influences on health care.
• Knowledge of state and federal home‑care regulatory guidelines (preferred).
• Experience in home health care preferred; experience with high‑acuity patients is a plus.
• Experience working with an interdisciplinary team (highly desired).
• Case management fundamentals (highly desired).
• Self‑directed, empathetic, critical thinker, outstanding interpersonal skills, leadership, organized.
