Hospice Admission Registered Nurse / RN

• Enable patients to spend quality time with their loved ones and do the activities they enjoy
• Assess patients for hospice eligibility and explain hospice care services to patients, families, and caregivers
• Ensure all patient needs are addressed initially
• Collaborate with the Inter-Disciplinary Team regarding the initial plan of care, patient condition, and other essential information

• RN license in the state you work
• Two years of experience as a registered nurse; hospice experience preferred
• Current driver’s license
• Current CPR and BCLS certification
