NHC Place Sumner is looking for a Social Worker to join our team! The qualified applicant for this position should hold a Bachelor's degree from an accredited program or a related degree. Experience preferred with a case management background in a SNF/ICF setting, knowledge of MDS and clinical assessments and documentation, care planning, discharge planning, working with support agencies such as hospice, home health and mental health services, etc.
Responsibilities- Exhibiting strong clinical and customer service skills in a fast-paced healthcare environment
- Working with individual patients and their representatives toward transition back to the community or adjustment to centre life and helping navigate crisis of illness and disability
- Serving in an advocacy role for patients and their representatives in expressing and resolving grievances
- Coordinating patient’s plan of care with interdisciplinary team members
- Reporting to the Director of Social Work current case management status and needs
- Maintaining confidentiality of professional information
