We're currently seeking a dedicated Registered Nurse (RN) to join our Home Health & Hospice Department in Galion, Ohio.
Position Overview
The Home Health and Hospice Registered Nurse provides nursing care to the Home Health and Hospice patient as prescribed by the Physician and requested by the patient/caregiver within the framework of the plan of treatment/care in compliance with applicable laws and regulations and the policies of the organization.
Responsibilities
- Visit patients in their homes for one-on-one visits
- Complete initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of home health patients
- Assess patients' conditions and chart observations, and perform evaluation tasks, including vital signs and medication review
- Administer medication as prescribed by the patient's physician
- Educate patients and their families on proper home health care strategies and procedures, and make recommendations for devices or tools that might improve the patient's quality of life
- Provide effective communication to patients/clients, their family members, team members, and other health care professionals
Required:
- Associate's Degree (A.D.), Diploma, Bachelor's Degree (B.S.), or Master of Science (M.S.) from an approved school of nursing.
- Current State of Ohio Registered Nurse licensure.
- BLS certification or obtained within 30 days of hire.
- Current valid Ohio driver's license.
- One year of nursing experience.
- Previous experience in community, health, home health, hospice, or appropriate specialty area.
