We’re currently seeking a dedicated Registered Nurse (RN) Case Manager to join our Home Health & Hospice Department in Galion, Ohio
.

The Home Health and Hospice RN – Case Manager is responsible for the overall care coordination of each home health or hospice patient covered by the agency from the time of referral to the time of discharge. They ensure timely delivery of appropriate services that promote quality and cost‑effective health care outcomes, working within established agency policies and Home Health and Hospice regulations.

The RN Case Manager works closely with visiting nursing staff, providing direct patient care to ensure that care is goal‑directed and patients’ needs are being met. The RN Case Manager acts as a patient advocate and primary contact for patients, caregivers, family, insurance companies, doctors, and staff to ensure effective communication regarding patient care.

• Associate’s Degree (A.D.), Diploma, Bachelor’s Degree (B.S.), or Master of Science (M.S) from an approved school of nursing.
• Minimum of two (2) years’ previous home health or hospice experience or in combination with supervisory experience.
• Current State of Ohio Registered Nurse licensure.
• BLS certification or obtained within 30 days of hire.
• Current valid Ohio Driver’s license.
