BAYADA Home Health Care is currently seeking an experienced Physical Therapist (PT) for a full-time opportunity performing outpatient home health visits for our senior living office. This office services adult and geriatric clients on a per-visit basis in Independent and Assisted Living Communities throughout Montgomery County.

As a home care Physical Therapist, you will be a vital member of a multidisciplinary healthcare team providing rehabilitative care and skilled nursing, helping clients receive the medical care needed to remain at home.

Responsibilities include:
- Making outpatient visits to clients in designated geographic areas.
- Performing diagnostic tests and measurements such as mobility, joint range of motion, transfer status, stability, ambulation patterns, muscle strength and endurance, balance testing, and safety assessments.
- Developing and implementing individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
- Assessing and revising care plans regularly and collaborating with nursing on multidisciplinary care planning.
- Educating clients, family members, or representatives on rehabilitative care and activities to promote health, safety, and independence.
- Documenting observations, interventions, and evaluations accurately using a state-of-the-art touchpad tablet.
Qualifications:
- Current MD Physical Therapist (PT) license.
- Minimum of one year recent experience as a Physical Therapist.
- Graduation from an American Physical Therapy Association-approved program or equivalent.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
