SummaryWe are seeking a caring and compassionate Licensed Practical Nurse (LPN) who will provide excellent direct care to our residents on a daily basis. The incumbent will perform assigned nursing procedures for the comfort and well‑being of residents such as assist in admission of new residents; assist with activities of daily living when needed; administer specified medications; supervise non‑licensed nursing staff and participate in the evaluation of non‑licensed staff.

- Sets and manages priorities.
- Accepts calls from staff during nights and weekend.
- Meets with oncoming and departing licensed practical nurse to discuss resident's condition and any occurrences from the shift.
- Responsible for making sure communications are clear, accurate and timely.
- Counts controlled substances with outgoing nurse, assumes responsibility for assuring accuracy of count before accepting keys.
- Checks, orders, and administers prescribed medication with 100% accuracy.
- Responsible for always ensuring the correct dosage by never pre‑pouring medications.
- Demonstrates knowledge of important medication interactions and side effects.
- Ensures medication cart is locked at all times.
- Obtains pertinent information and completes all required forms as established by policy and procedure and ongoing thereafter, upon resident admission.
- Responsible for completing and maintaining the ECC and LNS logs, service plan, progress notes, monthly summaries and follows healthcare providers orders.
- Assists in completing evaluations when needed for residents.
- Provides hands on care on an as‑needed basis to meet the needs of the residents.
- Works with the ADORS in reviewing and investigates all incidents reports and recommends appropriate action and follow up to the DORS/executive director.
- Understands and adheres to the ALF regulations pertaining to assisted living health care laws.
- Maintains a safe and secure working environment free of objects/situations that could cause harm and/or falls to residents, families, visitors and associates.
- Responsible for always practicing good body mechanics and safe working habits.
- Knows, understands, and follows established policy and procedures in emergency situations.
- Takes time to give associates in nursing daily feedback on his/her performance.
- Participates in the associates performance evaluations when requested by the DORS.
- Responsible for ensuring associates participate in activities provided to residents as described in the activities calendar.
- Responsible for always responding promptly and courteously to resident's request for assistance including phone calls and requests from family and visitors.
- Ensures there is a safe, clean and orderly environment for all residents.
- Responsible for always ensuring and supporting a dignified and caring atmosphere for residents, families, visitors and associates.
- Responsible for always wearing clean and appropriate dress per established policy.
- Understands and complies with HIPAA regulations.
- Responsible for being considered essential personnel especially during emergencies.
- Takes the initiative in work, seeks out additional information and offers solutions/suggestions.
- Performs all other reasonable duties as assigned or as requested.
- Responsible for willingly taking on special projects or assignments outside of regular duties.
