Summary

We are seeking a caring and compassionate Licensed Practical Nurse (LPN) who will provide excellent direct care to our residents on a daily basis. The incumbent will perform assigned nursing procedures for the comfort and well‑being of residents such as assist in admission of new residents; assist with activities of daily living when needed; administer specified medications; supervise non‑licensed nursing staff and participate in the evaluation of non‑licensed staff.

Duties and Responsibilities
  • Demonstrates excellent decision‑making skills and can understand the impact of those decisions.
  • Sets and manages priorities.
  • Accepts calls from staff during nights and weekend.
  • Meets with oncoming and departing licensed practical nurse to discuss resident's condition and any occurrences from the shift.
  • Responsible for making sure communications are clear, accurate and timely.
  • Counts controlled substances with outgoing nurse, assumes responsibility for assuring accuracy of count before accepting keys.
  • Checks, orders, and administers prescribed medication with 100% accuracy.
  • Responsible for always ensuring the correct dosage by never pre‑pouring medications.
  • Demonstrates knowledge of important medication interactions and side effects.
  • Ensures medication cart is locked at all times.
  • Obtains pertinent information and completes all required forms as established by policy and procedure and ongoing thereafter, upon resident admission.
  • Responsible for completing and maintaining the ECC and LNS logs, service plan, progress notes, monthly summaries and follows healthcare providers orders.
  • Assists in completing evaluations when needed for residents.
  • Provides hands on care on an as‑needed basis to meet the needs of the residents.
  • Works with the ADORS in reviewing and investigates all incidents reports and recommends appropriate action and follow up to the DORS/executive director.
  • Understands and adheres to the ALF regulations pertaining to assisted living health care laws.
  • Maintains a safe and secure working environment free of objects/situations that could cause harm and/or falls to residents, families, visitors and associates.
  • Responsible for always practicing good body mechanics and safe working habits.
  • Knows, understands, and follows established policy and procedures in emergency situations.
  • Takes time to give associates in nursing daily feedback on his/her performance.
  • Participates in the associates performance evaluations when requested by the DORS.
  • Responsible for ensuring associates participate in activities provided to residents as described in the activities calendar.
  • Responsible for always responding promptly and courteously to resident's request for assistance including phone calls and requests from family and visitors.
  • Ensures there is a safe, clean and orderly environment for all residents.
  • Responsible for always ensuring and supporting a dignified and caring atmosphere for residents, families, visitors and associates.
  • Responsible for always wearing clean and appropriate dress per established policy.
  • Understands and complies with HIPAA regulations.
  • Responsible for being considered essential personnel especially during emergencies.
  • Takes the initiative in work, seeks out additional information and offers solutions/suggestions.
  • Performs all other reasonable duties as assigned or as requested.
  • Responsible for willingly taking on special projects or assignments outside of regular duties.
Supervisory Responsibilities
