Summary

We are seeking a caring and compassionate licensed practical nurse (LPN) who will provide excellent direct care to our residents on a daily basis. The incumbent will perform assigned nursing procedures for the comfort and well‑being of residents such as assisting with admissions, assisting with activities of daily living when needed, administering specified medications, supervising non‑licensed nursing staff, and participating in the evaluation of non‑licensed staff.

Duties and Responsibilities
  • Demonstrates excellent decision‑making skills and can understand the impact of those decisions.
  • Sets and manages priorities.
  • Accepts calls from staff during nights and weekends.
  • Meets with oncoming and departing LPN to discuss resident's condition and any occurrences from the shift.
  • Responsible for making sure communications are clear, accurate and timely.
  • Counts controlled substances with outgoing nurse, assumes responsibility for assuring accuracy of count before accepting keys.
  • Checks, orders, and administers prescribed medication with 100% accuracy.
  • Ensures the correct dosage by never pre‑pouring medications.
  • Demonstrates knowledge of important medication interactions and side effects.
  • Ensures medication cart is locked at all times.
  • Obtains pertinent information and completes all required forms as established by policy and procedure upon resident admission.
  • Responsible for completing and maintaining the ECC and LNS logs, service plan, progress notes, monthly summaries and follows healthcare providers orders.
  • Assists in completing evaluations when needed for residents.
  • Provides hands‑on care as needed to meet resident needs.
  • Works with the ADORS in reviewing and investigating all incident reports and recommends appropriate action and follow‑up to the DORS/executive director.
  • Understands and adheres to the ALF regulations pertaining to assisted living health care laws.
  • Maintains a safe and secure working environment free of objects/situations that could cause harm and/or falls to residents, families, visitors and associates.
  • Practices good body mechanics and safe working habits.
  • Knows, understands, and follows established policy and procedures in emergency situations.
  • Takes time to give associates in nursing daily feedback on performance.
  • Participates in associate performance evaluations when requested by the DORS.
  • Ensures associates participate in activities provided to residents as described in the activities calendar.
  • Responds promptly and courteously to resident's requests for assistance, including phone calls and requests from family and visitors.
  • Ensures there is a safe, clean and orderly environment for all residents.
  • Supports a dignified and caring atmosphere for residents, families, visitors and associates.
  • Wears clean and appropriate dress per established policy.
  • Complies with HIPAA regulations.
  • Is considered essential personnel especially during emergencies.
  • Takes initiative in work, seeks additional information, and offers solutions or suggestions.
  • Performs all other reasonable duties as assigned or requested.
  • Willingly takes on special projects or assignments outside regular duties.
Supervisory Responsibilities

Directly supervises employees in the Resident Services/Nursing Department. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.
