The Children's First program serves children from birth to 5 years old who have special needs. We provide screening for developmental concerns, in which we can evaluate physical and cognitive health, motor skills, communication, social/emotional growth, and adaptive development.

Children’s First Community Health Worker within the Children’s with Special Needs Division at District Two, Georgia Department of Public Health. You will join a dedicated group of public health professionals that help support the overall mission and vision of protecting lives of Georgian residents in a variety of ways. District 2 Public Health serves 13 counties in northeast Georgia.

Under general supervision, you will provide a broad range of paraprofessional social services and/or community health services for infants, children and families in a specified geographic area. You will provide developmental screenings, patient education, and make necessary referrals for other social services. You may perform case management duties.

Job Duties
  • Provide developmental screening (Ages & Stages; MCHAT‑R) to all children, birth to age five years, as assigned by District Children 1st Coordinator or designee.
  • Send ASQ online link and enter developmental screenings into ASQ online if necessary.
  • Provide home visits to include assessments, parent education, developmental screening and activities according to Children 1st model of service delivery.
  • Refer infants and children to public health (BCW, CMS, EHDI, WIC, etc.) and community programs as appropriate.
  • Ensure that all children receiving services are linked to a primary healthcare provider.
  • Assure that all identified families receive Children 1st Maternal Child health (MCH) Integrated assessments according to programmatic guidelines.
  • Complete all necessary documentation, both written and electronic, and reporting requirements by specified deadline or as determined by Children 1st District Coordinator.
  • Enter follow up actions, assessment, etc. into SENDSS NB.
  • Upload all appropriate forms into SENDSS & scan to archives.
  • Provide correspondence and/or referral status letter to DFCS, PCPs, and other stakeholders according to confidentiality and privacy laws (District 2 policy; HIPAA; FERPA; IDEA).
  • Assist with data entry, scanning & filing as needed and/or assigned.
Qualifications

High school diploma/GED and two (2) years of job‑related experience in a social services‑related position; or one (1) year of experience at the lower level Social Services Tech 1 (SST
010) or position equivalent.

  • Bachelor’s degree in a related field and one to two years’ experience in providing comprehensive in‑home assessments for mothers, infants, and children, or work experience in social services delivery (case management, outreach, prevention/education, etc.).
