Schedule:
8 Hr Morning - Afternoon
Performs a wide range of support services for the Case Management staff, assists RN Case Manager and Social Worker with discharge planning, continuum placement, communication with insurance companies, data gathering. Collaborates with physicians and team, supports patient care monitoring, coordination and facilitation. Promotes quality outcomes, team accountability, productivity, and serves as a link among staff, patients, providers, payors and community resources.
Demonstrates strong communication skills and judgment. Interacts with all age groups. Performs clinical duties per population guidelines and National Patient Safety Goals. Provides cross coverage, including weekend rotation. Follows identified patients post-discharge.
- Educational Requirements:
High School Diploma or GED - Minimum Experience:
Two (2) years of healthcare experience
- Preferred Licensure:
Current Georgia LPN license - Preferred
Educational Requirements:
Licensed Practical Nurse with an active Georgia license or Associate’s Degree in Health or Human Services
Skills And Abilities

- Sense of urgency while mindful of financial stewardship
- Supports collaborative practice environment using a team approach, enhances continuity of patient care, documents activities timely.
- Responds to referrals same day, monitors length of stay, conducts documentation review.
- Seeks nursing home placement promptly, obtains post-acute authorizations, arranges discharge services per physician orders.
- Completes transfer and DMA-6 forms, supplies information to nursing homes and agencies, follows up for placement success.
- Arranges DME, home health, and transportation; advocates for patient; assists navigation of healthcare delivery system.
- Facilitates communication among patient, family, providers, and post-acute providers.
- Works scheduled shifts including weekends and remote coverage.
- Promotes positive work culture, engages staff, and participates in growth opportunities.
- Other duties as assigned; follows identified patients post-discharge to mitigate readmission.
