As a Registered Nurse, you will be responsible for delivering high‑quality, patient‑centered care in line with the requirements of the department and the standards of practice for the relevant state and specialty. Collaborating with medical providers and the care team, you will provide personalized, comprehensive, and compassionate care, following established nursing models such as "Assess, Perform, Teach, and Manage." You will also act as an advocate for patients, families, and caregivers, embodying the organization's vision, mission, and values to ensure an outstanding patient experience and positive clinical outcomes.

Monday–Friday Days. Senior Healthcare Centers are outpatient departments of HCA Florida North Florida Hospital. Our mission is to provide exceptional primary care to seniors. Through our dynamic interdisciplinary team of physicians, nurses, case managers, pharmacists, lab technicians, and support staff, Senior Healthcare Centers address the wide range of medical and psychosocial issues affecting the aging population.  Our patient appointments include annual physicals, follow‑up care for chronic disease monitoring, work‑in acute issue management, hospitalization follow‑up, lab draw, nursing follow‑up care, etc.

• Collect vital signs, pain scale, chief complaint, assessment questionnaire.
• Medication reconciliation review.
• Coordinate flow of patient clinic for provider.
• Completes and/or coordinates treatment orders prescribed for the MD during the visit (i.e. vaccination, medication administration, venipuncture, EKG, bladder scan, ear lavage, consult with pharmacist or case manager).
• Documentation within electronic health record (eClinical
  
  Works).
• Review discharge instructions with patient and/or family.
• Appropriately direct patient inquiries in person and on the phone.
• Manage multiple priorities.
• Collaboration with other Senior Healthcare Centers and hospital department team members.
