The RN Case Manager assesses, plans, implements and monitors a comprehensive plan of care for a group of patients. This is done in collaboration with other members of the medical team, including physicians, social workers, PT/OT staff, pharmacists, respiratory, dietary, and nursing. The RN maintains focus on the patient\u201∙ progression of care throughout the hospitalization, works with the care team on appropriate resource utilization, and creates a safe, effective, and efficient transition and discharge from the hospital.

• Licensed Registered Nurse (RN) in Florida with a valid license.

• At least two years of recent RN experience in a clinical capacity applicable to the designated population served.
• Prior Case Management experience preferred.
• Professional nursing board certification in specialty area preferred; certification required within two years of employment for those hired after January 1, 2018.
• Motor Vehicle Operator designation:
  Employees in this position will not operate vehicles for an assigned business purpose. A frequent driver is defined as one who uses personal or Shands automobile at least once daily, at least five individual trips per week, or drives, on average, over 150 miles per week in the performance of their job.
