Overview
The RN Case Manager assesses, plans, implements, and monitors a comprehensive plan of care for her/his group of patients. This is done in collaboration with other members of the medical team, including physicians, social workers, PT/OT staff, pharmacists, respiratory, dietary, and nursing. The focus is on the patient’s progression of care throughout the hospitalization, working with the care team on appropriate resource utilization and creating a safe, effective, and efficient transition/discharge from the hospital.
ResponsibilitiesAssess, plan, implement, and monitor a comprehensive plan of care for patients. Collaborate with the interdisciplinary team to ensure appropriate resource utilization. Facilitate a safe and effective transition/discharge from the hospital.
QualificationsLicensed RN in the State of Florida. BSN preferred. Two years of recent RN experience in a clinical capacity applicable to the designated population served. Prior case management experience preferred. Case managers hired after January 1, 2018 who do not possess a BSN and/or a MSN must obtain a BSN within three years of employment. Professional nursing board certification in a specialty area preferred.
Case managers hired after January 1, 2018 must obtain specialty certification within two years of employment. Valid RN license in Florida. Motor Vehicle Operator designation: individuals in this position will not operate vehicles for an assigned business purpose.
