Physical Therapist (PT) - Per Diem, PRN

Parklands Care Center is seeking a dedicated and enthusiastic Physical Therapist (PT) to join our team full-time. In this rewarding role, you will provide comprehensive rehabilitation services to residents in our subacute rehabilitation unit, helping them regain mobility and enhance their quality of life.

• Patient Assessment: Conduct thorough evaluations to assess patients’ physical capabilities, limitations, and rehabilitation needs.
• Individualized Treatment Plans: Develop and implement tailored treatment plans based on each resident's specific goals and needs.
• Therapeutic Interventions: Utilize a variety of therapeutic techniques, exercises, and modalities to improve strength, mobility, and functional independence.

  Educate residents and their families about rehabilitation processes, exercises, and strategies for preventing injuries.
• Collaboration: Work closely with a multidisciplinary team, including occupational therapists, speech-language pathologists, nurses, and physicians, to provide comprehensive patient care.
• Progress Monitoring: Regularly evaluate patient progress and modify treatment plans as necessary to ensure optimal outcomes.
• Documentation: Maintain accurate and timely documentation of evaluations, treatment plans, and progress notes in accordance with regulatory standards.

• Licensure: Current state licensure or eligibility for licensure as a Physical Therapist in Florida.
• Experience: Previous experience in a rehabilitation or healthcare setting is preferred, but new graduates are encouraged to apply.
• Skills: Excellent communication and interpersonal skills; a passion for patient care and teamwork.
