Schedule: Monday – Friday, 40 hours per week

• Evaluate, plan, and implement individualized treatment programs for patients in an outpatient setting.
• Utilize evidence‑based practices to help patients regain strength, mobility, and independence.
• Collaborate with referring physicians and interdisciplinary teams to ensure optimal patient outcomes.
• Maintain accurate and timely documentation of patient progress and treatment plans.
• Provide education and support to patients and their families regarding home exercise programs and wellness.

• Education:
  
  Graduate of an accredited Physical Therapy program.
• Licensure:
  Current Physical Therapist license in the state of Kentucky (or eligibility to obtain).
• Experience:
  
  Minimum 1 year of outpatient physical therapy experience required.
• Certifications:
  
  BLS certification required.
