• Competitive compensation: $145,399 – $174,478 with quarterly bonus

We are seeking a licensed Nurse Practitioner or Physician Assistant with at least two years of experience working with the elderly. Must have prescriptive authority, DEA registration, and be eligible to serve Medicare/Medi‑Cal participants. Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred.

Salary/Wage base range: $145,399 – $174,478 per year + bonus. Salary will be adjusted based on experience and other qualifications.
