Hospice Nurse Practitioner (Part-Time, Travel Assignment) Fresno, CA Join our compassionate team as a Hospice Nurse Practitioner and make a meaningful difference in patients' lives through personalized end-of-life care. Required Skills:
Valid DEA number and current Nurse Practitioner license Proven experience in hospice or palliative care Strong assessment and clinical decision-making skills Excellent communication and interpersonal skills Ability to work independently with minimal supervision Proficient in electronic medical records (EMR) systems Knowledge of Medicare recertification processes and face-to-face evaluation requirements Nice to Have
Skills:
Experience working in skilled nursing facilities and home health settings Bilingual abilities to serve diverse patient populations Certification in palliative or hospice care preferred Familiarity with care coordination and interdisciplinary team collaboration Preferred
Education and Experience:
Master's degree or higher in Nursing (NP certification required) At least 2 years of clinical experience in hospice, palliative, or related fields Previous travel or home health experience is a plus Other Requirements:
Must have reliable transportation; ability to travel locally within Fresno and surrounding areas Flexibility to work 1 2 days per week, with full or half-day arrangements Workday hours are daytime, Monday through Friday; evenings and weekends are not required Ability to conduct face-to-face evaluations for Medicare recertifications Effective communication with clinical teams and patients families This is an exceptional opportunity to join a dedicated team committed to delivering compassionate care while enjoying flexible scheduling and competitive hourly pay of $81.74.
