Community Health Partners is seeking an Advance Practice Provider (APP) to work in an established Maternal Fetal Medicine practice. The APP will provide compassionate, personalized care for high-risk obstetric patients in collaboration with Maternal Fetal Medicine physicians.

• Completion of a Nurse Practitioner or Physician Assistant program required
• Credentialed by the CMC Medical Staff Office as an Allied Health Professional and privileged to perform Nurse Practitioner or Physician Assistant Standardized Procedures required

• 2 years of clinical experience in the area of specialty preferred

• If qualifying as a Nurse Practitioner, the following is required:
• NP - Current State of California Nurse Practitioner License required
• NPF - Nurse Practitioner Furnishing Number required
• If qualifying as a Physician Assistant, the following is required:
• PA - State of California Physician Assistant License required
• DEA - DEA Controlled Substance Number required
• BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
• Ability to participate in a federally funded program (Medicare and Medicaid) required
• NPC - National Certified Nurse Practitioner by ANCC (American Nurses Credentialing Center) or AANP (American Academy of Nurse Practitioners) preferred
• CRNFA - Certified RN First Assistant preferred
