• Provide direct patient home care visits, observe, perform and report preventative, treatment and rehabilitative nursing procedures while implementing the plan of care
• Observe and report on the home environment and make recommendations that will enhance patient care, promote safety and prevent adverse effects
• Work with patient, family, and health care team members to coordinate services, promote quality of care and positive patient care outcomes
• Take verbal orders from Health Care Providers as necessary and within the LVN scope of practice in a timely manner and document appropriately
• Inform Health Care Providers of changes in patient’s condition in accordance with providers orders and document communications accordingly
• Provide care as ordered that is appropriate preventive, rehabilitative, therapeutic, comfort and pain interventions
• Provide instructions to patient/CG’s and staff in a timely manner

• A compassionate and dependable Licensed Vocational Nurse (LVN) with experience or interest in home health nursing
• Comfortable providing skilled nursing care in the home setting while working independently
• Skilled at building meaningful relationships with patients and caregivers
• Committed to delivering high‑quality care to patients recovering from illness, surgery, or injury, as well as those managing chronic conditions
• Motivated by improving patient outcomes, quality of life, and reducing avoidable hospitalizations
• Organized, communicative, and confident in patient education and documentation

• Graduate of an accredited Licensed Vocational Nurse (LVN) nursing program
• Current, active California LVN license and CPR certification
• Minimum of one (1) year clinical nursing experience; home health experience preferred
• Ability to provide skilled nursing care in the home, including assessments and patient education
• Valid driver’s license, reliable vehicle, and current auto insurance (required for home visits)
• Strong communication, documentation, and organizational skills
• Experience with home health documentation systems preferred (HCHB a plus)
