About the Position
Join Our Team as a Hospice Nurse Practitioner – Pediatric Hospice Focus
Are you a compassionate and experienced Nurse Practitioner passionate about providing quality palliative care to pediatric patients? We are seeking a dedicated Hospice Nurse Practitioner to join our team on a part-time travel basis, making a meaningful difference in the lives of children and families in need of hospice services. Key Responsibilities:
Conduct hospice home visits (4–5 visits daily), ensuring compassionate care tailored to pediatric patients.
Provide care in skilled nursing facilities (SNFs) and other care settings as needed.
Perform face-to-face evaluations required for Medicare recertifications in collaboration with the Medical Director.
Communicate effectively with Case Managers for ongoing patient follow-up and coordination.
Offer educational support to clinical team members to promote best practices.
Participate in inpatient roundings when necessary to ensure patient well-being.
Required Skills:
Valid Nurse Practitioner (NP) license with pediatric hospice experience.
Proven ability to perform comprehensive hospice assessments and face-to-face Medicaid and Medicare recertification evaluations.
Strong communication skills, both verbal and written.
Ability to manage a flexible schedule and travel to patient locations.
Compassionate nature with a commitment to patient-centered care.
Nice to Have
Skills:
Experience working with multidisciplinary teams.
Familiarity with electronic medical records (EMR) systems.
Prior home health or hospice experience beyond pediatrics.
Preferred
Education and Experience:
Master’s degree in Nursing (MSN or higher).
2+ years of pediatric hospice or palliative care experience.
Certification in hospice and palliative nursing is a plus.
Other Requirements:
Valid state licensure as a Nurse Practitioner.
Ability to travel locally to various patient sites.
Commitment to maintaining all required certifications and licensure.
No evenings or weekends; scheduled Monday–Friday shifts, flexible full- or half-day options.
