This is for inpatient and 4‑5 home visits per day. This role offers exceptional schedule flexibility and meaningful work supporting hospice and palliative patients across care settings.

  Fresno, CA (based out of the Fresno location)
• Shift: 8 Hour Days
• Pay: $90 p/h + Mileage
• Start Date:
  
  ASAP
• Schedule:
  
  2 days per week. Facility is highly flexible and can accommodate full days or half days

• Assist the Medical Director with face‑to‑face evaluations for Medicare recertification
• Communicate with the Case Manager (CM) when patients require additional follow‑up
• May round in the inpatient unit
• May provide home visits to Skilled Nursing Facilities (SNFs), Care facilities, Private homes
• Educate clinical team members as needed
• Additional responsibilities to be discussed during interview
