As a Home‑Health RN, you will develop a plan of care in collaboration with patients, family or caregivers, physicians, and other disciplines. You will visit patients’ homes to monitor their conditions and provide skilled nursing care.

• Perform comprehensive assessments in patients’ homes and identify patient needs.
• Develop a care plan with patient, caregiver, healthcare provider, case manager, and/or supervisor and document appropriately.
• Assess the need for additional services and equipment on admission and inform the case manager and/or supervisor in a timely manner.
• Assess the home environment and recommend changes to enhance patient care, promote safety, and prevent adverse effects.
• Take verbal orders from the healthcare provider and document promptly.
• Initiate appropriate preventative, rehabilitative, therapeutic, comfort, and pain interventions and provide instructions to patients and caregivers.
• Maintain high standards of clinical care and stay current on clinical and medication issues.
• Update patients’ clinical records, including medication profiles, plan of care, OASIS, case conferences, progress notes, and re‑certification documentation as required.
• Serve as a preceptor when needed.
• Other duties as assigned.

You are experienced and ready to bring your skills to patients who need them most. You excel in building meaningful relationships, thrive in a home setting, and deliver high‑quality nursing care independently.

• Graduation from an accredited nursing program.
• Current state RN license and CPR certification.
• At least one year of clinical practical nursing experience outside of school/rotations;
  Home Health experience preferred.
• Must have a vehicle, current driver’s license, and automobile insurance.
• Strong interpersonal and communication skills.
• Ability to make home visits and provide skilled nursing assessments and care.
• Experience with Home Health and Home Care Home Base (HCHB) documentation preferred.
