Summary
The Community Nutritionist plays a key role in advancing whole-person, preventive, and lifestyle-focused care within the community. Delivers culturally appropriate evidence-based nutrition and wellness education/coaching; collaborates closely with interdisciplinary care teams to address chronic disease prevention, management, and health equity through nutrition and lifestyle interventions. Provides clinical oversight and quality assurance for Medically Tailored Meal (MTM) program; reviews documentation and assessments completed by supervised staff for quality and accuracy;
trains staff on MTM guidelines, lifestyle intervention protocols, materials, presentations, and workflow processes; supports onboarding of new staff and provides ongoing coaching and performance feedback; assists leadership in developing protocols, workflows, and quality improvement processes; provides oversight and evaluation of Nutrition Interns. Assist in the fulfillment of the Agency’s vision, “A strong Fresno County where people have resources to shape their future free from poverty.”
And Responsibilities/Essential Functions
- Provides clinical oversight and quality assurance for MTMs, ensuring alignment with patient diagnoses, evidence-based nutrition standards, and program requirements.
- Collaborates with care teams to identify eligible participants and integrate nutrition services into care plans based on clinical criteria and program guidelines.
- Conducts comprehensive nutrition assessments and education/coaching using medical history, diagnoses, lab values, anthropometrics, medications, psychosocial factors, and functional status.
- Delivers evidence-based Medical Nutrition Therapy (MNT) to individuals and groups, addressing chronic disease prevention and management (e.g., diabetes, cardiovascular disease, obesity, hypertension).
- Develops and maintains accurate clinical documentation within established time frames, in accordance with organizational policies, regulatory standards, and billing requirements.
- Provides Lifestyle Medicine-based nutrition and behavior change coaching using motivational interviewing and patient-centered approaches that are culturally sensitive, accessible, and individualized.
- Contributes to program evaluation and continuous quality improvement by supporting patients in adopting sustainable health behaviors aligned with Lifestyle Medicine principles and monitoring outcomes.
- Plans and delivers community-based nutrition and lifestyle education presentations, workshops, and group classes.
- Partners with community organizations to support food access and health promotion initiatives.
- Researches and compiles data and information for use in various grants and reports.
- Collaborates in interdisciplinary case conferences and coordinated care team meetings.
- Communicates nutrition findings, recommendations, and progress updates to the broader care team.
- Ensures compliance with HIPAA, organization policies, state regulations, and payer requirements.
- Maintains an accurate record keeping and filing system for all project components.
- Responsible for supervising interns and volunteers.
- Travels for agency business using reliable transportation.
- Maintains regular attendance and punctuality.
- Works evenings and weekends as required.
- Performs other duties as assigned.
Education
- High school diploma or GED.
- Bachelor’s degree in Nutrition, Dietetics, or related field. Master’s degree preferred.
Experience
- Registered Dietitian Nutritionist (RDN), current and in good standing is required.
- Experience working with underserved population in community/public health settings.
- Familiarity with MTM programs and Food Is Medicine initiatives.
Other Qualifications
- Certification in Lifestyle Medicine through the American College of Lifestyle Medicine (ACLM) required, or willingness to obtain within the first year of employment.
- Certification in Lifestyle Coaching through nationally recognized authority is preferred.
- Proven patient-centered, culturally responsive care; bilingual or multilingual skills preferred.
- Proficient in use of Electronic Health Records, Microsoft Office Suite and related software.
