Must have Hospice experience. Palliative NP needed for 1-2 days/week WITH PEDS experience.

Will communicate with the CM when any patient requires additional follow up. May round in the inpatient unit. May provide home visits to SNF, care facilities or homes.

May educate clinical team members regarding care as needed.
