Case Manager Full Time – San Joaquin Valley Rehabilitation Hospital

Join our team as a Case Manager at San Joaquin Valley Rehabilitation Hospital, a 62‑bed acute rehab facility in Fresno, CA. We provide rehabilitative services to people recovering from disabilities caused by injuries or illnesses, helping patients regain physical or cognitive abilities and return home at their highest level of independence.

- Coordinate clinically complex patient care across a continuum, ensuring and facilitating the achievement of quality clinical and cost outcomes.
- Negotiate, procure, and coordinate appropriate services and resources needed by patients, intervening as necessary to address and resolve issues.
- Ensure appropriate sequencing of treatment goals and implement a comprehensive discharge plan.
- Demonstrate knowledge and skills necessary to provide case management services to the patient population experiencing rehabilitation needs.
- Current, valid, and active license to practice as a Registered Nurse, Occupational Therapist, Physical Therapist, Speech Therapist, or Licensed Social Worker in the state of employment.
- Current BLS certification from a Vibra‑approved vendor.
Qualifications & Skills
- Minimum one (1) year of case‑management experience (preferred).
- Previous rehabilitation experience (preferred).
- CCM certification strongly preferred; ACM, CRRN, or CIRS preferred.
- Working knowledge and ability to apply professional standards of practice in job situations.
- Assertive and personable with the ability to balance complex quality, cost‑appropriate, and service‑oriented outcomes.
- Comprehensive knowledge of the principles and concepts of case management and applicable standards of voluntary and regulatory agencies.
- Proficient in collecting and retrieving material from medical records.
- Knowledgeable in utilization review, medical terminology, appropriate levels of care, treatment modalities, statutes, and healthcare delivery models.
- Excellent written and oral communication skills with internal and external stakeholders.
- Highly refined interpersonal relationship abilities, negotiation, and problem‑solving skills.
- Strong clinical assessment and process skills; demonstrated critical thinking in all situations.
- Professional image, strong organizational, prioritizing, and analytical skills.
- Independent decision‑making ability when warranted.
- Proficiency with computer and software applications used in job functions.
- Freedom from illegal drug and alcohol use in the workplace.

CompensationThe wage range for this role is USD $37.00 - $50.80 per hour, reflecting skills, experience, education, licensure, and other business factors. Compensation decisions are based on each case’s circumstances.
