We are currently seeking a Registered Nurse to service in Stockton and surrounding areas. If you have acute care experience, our hourly rate ranges from $47 to $55 with negotiable factors such as experience and availability. Salary options and overtime opportunities are also available.

Essential Job Functions / Responsibilities

• Assess the psychosocial status of patients related to the patient’s illness and environment and communicate findings to the registered nurse.
• Carry out social evaluations and plan interventions based on findings.
• Maintain clinical records on all patients referred to social work.
• Prepare clinical and progress notes.
• Provide information, referral services, and application assistance for community resources (e.g., Medi-Cal, IHSS) to patients and families/caregivers.
• Serve as liaison between patients/families and community agencies.
• Maintain collaborative relationships with organization personnel to support patient care.
• Develop and maintain contracts with public and private agencies as resources for patient and organization personnel.
• Participate in the development of the total plan of care and case conferences as required.
• Assist physicians and other team members in understanding significant social and emotional factors related to health problems.
• Participate in discharge planning.
• Perform other duties as delegated by the Clinical Director/Supervisor.

Position Qualifications

• Graduate of a Master's program in social work accredited by the Council on Social Work Education (doctoral degree preferred).
• Active ACSW registration with the California Board of Behavioral Sciences (BBS) required.
• Minimum of one (1) year’s experience in health care.
• Experience in home health care preferred.
• Excellent verbal and written communication and organization skills.
• Current CPR certification.
• Licensed driver with a reliable automobile that is insured per organization requirements.
• Proficiency in word processing, spreadsheets, and email, and basic computer operation.
• Familiarity with resource linkage to local and state agencies.
