Description
GENERAL DESCRIPTION OF THE POSITION:
The Case Manager at Tatum Psychology Employment Group is responsible for overseeing documentation of patient’s intake, progress, and discharge, as well as linking them with appropriate social, occupational, and educational resources as needed. The Case Manager works well with the treatment team to plan and coordinate the client’s progress through the continuum of care.
Schedule:
Tuesday - Saturday 2:00pm-10:30pm
ESSENTIAL FUNCTIONS:
1. Patient Admission & Orientation
- Admits new patients by completing the intake process and new patient orientation.
2. Assessment & Treatment Planning
- Evaluates patients’ needs for medical, psycho-social, and psychiatric treatment.
3. Case Management & Patient Monitoring
- Monitors cases by verifying patients’ attendance, observing, and evaluating progress in treatment; advocating for needed services; obtaining additional resources; providing personal support.
- Maintain patients’ records by reviewing case notes and documenting progress.
4. Communication & Interdisciplinary Collaboration
- Communicates patients’ progress by participating in weekly interdisciplinary meetings and evaluations; disseminating results and obstacles to therapeutic team and family; identifying treatment influences.
5. Discharge Planning & Post-Care Coordination
- Prepares patients’ discharge by creating discharge plans; coordinating discharge and post-discharge requirements; orienting and training family members; providing resources.
- Prepares reports by collecting, analyzing, and summarizing treatment and results data and trends; compiling statistics.
6. Staff Development & Training
- Improves treatment results by studying, evaluating, and re-designing processes; implementing changes; rewriting policies and procedures.
- Improves staff competence by providing educational resources; balancing work requirements with learning opportunities; evaluating the application of learning to changes in treatment results.
- Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.

Requirements
MINIMUM QUALIFICATIONS:
EXPERIENCE:
Two (1) years’ experience in a behavioral health care setting.
SKILLS REQUIRED:
- Experience working with culturally diverse individuals and communities or have otherwise demonstrated a commitment to strengthening engagement of a diverse community and skill in communicating with diverse populations.
- Familiarity with equity and/or diversity initiatives within an organization.
