JOB SUMMARY

The Dual Diagnosis Mental Health Specialist/Case Manager IV reports to the Supervising Personal Service Coordinator and works as part of a treatment team in achieving the goals and objectives of the program to ensure quality of care for client participants.

ESSENTIAL JOB RESPONSIBILITIES
  • Under the supervision of the Supervising Personal Service Coordinator, assist in the accomplishment of the case management plan with clients by screening, evaluating, identifying, and implementing specific case management activities as identified in the Individual Service and Support Plan (ISSP) to support clients in achieving their goals.
  • Maintain appropriate case recording documentation as per the Rehab option Medi-Cal guidelines, case recording, and other contract guidelines.
  • Attend weekly supervision meetings with the Supervising Personal Service Coordinator.
  • Provide direct services, including case management, to clients. Such services may include group‑counseling, individual, and or other forms of direct service as outlined in the Individual Service and Support Plan (ISSP) and Plan of Care.
  • Maintain and submit all documentation and billing as it relates to client services in a timely fashion.
  • Staff expectations are to meet established performance standards of direct services to our individuals served while ensuring contacts are met per the level of care/treatment.
  • Plan, coordinate, and facilitate services for Severely Mental Ill (SMI) clients.
  • Screen and evaluate potential clients for the identification of case management, rehabilitation, residential, and other services and activities.
  • Provide educational groups on recovery, wellness and sobriety skills.
  • Develop and maintain outside supportive referral services for clients.
  • Evaluate and refer clients as needed to detoxification services, residential treatment services and sober living placements.
  • Provide individual counseling with clients assisting them in the development of sobriety skills.
  • Communicate closely, in a systematic fashion, with agencies and significant others involved in the rehabilitation of clients.
  • Provide the Supervising Personal Coordinator with timely reports on any unusual occurrences that might impact the overall program operation or the health and safety of clients or employees.
  • Consistently meet program performance standards including a service impact, sign and submit documentation goals, and attainment of required hours of job‑related in‑service training per year.
  • Complete the minimum training hours annually per Turning Point training requirements.
  • Think and act quickly and efficiently in emergencies.
  • Will drive on company business and transport consumers as needed.
  • Physical presence in the office is required.
ESSENTIAL JOB REQUIREMENTS
  • A bachelor’s degree from an accredited or regionally approved college or university in the area of social work, psychology, vocational rehabilitation, criminal justice, or a related field that is directly related to the specific responsibilities of the job.
  • At least one year of experience in a mental health or human services related field.
  • Preferred California Alcohol and Drug Programs (ADP) approved substance abuse counseling certification or enrollment in an ADP approved substance abuse counseling certification course within six (6) months of date of hire and successfully complete the course in a timely manner or at the discretion of the program supervisor.
  • Knowledge of substance abuse, addictions, and related disorders, and ability to effectively provide educational groups regarding the treatment of these disorders.
  • Knowledge of best practices regarding the treatment of clients with Severe Mental Illness (SMI) and Dual‑Diagnosis individuals.
  • Communicate effectively in written and spoken English.
  • Ability to operate within Electronic Medical Records (EMR) system.
  • Must be available to work all shifts as scheduled.
