Responsibilities- Provide personal care and escort support based on individualized treatment plans, including assistance with grooming, bathing, toileting, dressing, feeding, mobility, and peri-care
- Observe and report changes in patient condition to the Operations Director or designated clinical staff
- Communicate proactively regarding patient health and safety concerns to relevant team members
- Utilize safe practices and proper body mechanics while adhering to universal precautions
- Interact with patients and their caregivers in a professional, respectful, and friendly manner
- Support patients in social and recreational activities, encouraging engagement and interaction
- Carry out therapeutic interventions directed by rehabilitation staff, including the use of mechanical assistance for transfers and mobility
- Occasionally accompany patients to external clinical appointments, when needed
- Assist with meal delivery and monitoring during mealtime in the center
- Perform other tasks or projects as assigned to support overall patient care
- Other duties as assigned
- High school diploma or equivalent
- HHA or CNA certification strongly preferred
- Proficiency in Hmong language required
- At least 6 months of experience as a Personal Care Assistant, Certified Nursing Assistant, or EMT preferred
- Must successfully complete client-approved Personal Care Assistant training upon hire
- Meet standardized competencies required for this role before working independently
- Medical clearance for communicable diseases and up-to-date immunizations prior to direct patient contact
- Reliable transportation, valid driver’s license, and state-required liability insurance
- At least 1 year of experience working with frail or elderly patients preferred (training will be provided if not present)
- Familiarity with safe personal care techniques for individuals with physical limitations
- Experience working independently and within a collaborative care team
This position is based on-site in a clinic setting, working closely with both patients and the care team in a supportive and community focused environment.
ScheduleFull Time Monday-Friday
