ESSENTIAL JOB RESPONSIBILITIES- Ensure all new referrals are logged and entered into the Electronic Medical Record (EMR) in a timely manner.
- Screen and triage all referrals.
- Ability to respond to crisis related calls, triage, and determine appropriate level of support.
- Ability to organize and prioritize work so as to perform assigned duties independently.
- Work closely with program staff to schedule assessment appointments within county timeliness requirements and prioritize access for urgent referrals.
- Determine and confirm eligibility of all new intakes prior to initial appointment.
- Maintain integrity of the clinical information entered into the EMR.
- Complete the disposition status on the Electronic Medical Record (EMR) System for all referrals, after determining eligibility for services.
- Ensure all logs and required reports are maintained and updated.
- Interact with community partners to receive and make referrals. Links those referrals that can be served in a lower level of care in the community such as to a primary care setting.
- Provide direct services, including case management and rehab services, to clients including providing intake support at the beginning of service and support with appropriate linkage to additional resources as needed.
- Provide direct services in the clinic, and in the community, including individual and group treatment, case management, and consultation, as necessary.
- Meet directly with clients to complete initial admission documentation and enter new program enrollment in the EMR system.
- Maintain appropriate documentation as per Medi-cal guidelines and other contract guidelines and submit all documentation and billing as it relates to client services in a timely manner.
- Employees shall follow the reporting requirements for incidents of Child Abuse, Elder Abuse, Adult Abuse, Dependent Adult Abuse, and Tarasoff warnings per California reporting laws.
- Attend and participate in staff meetings and staff training.
- Will think and act quickly and efficiently in emergencies.
- Will drive on Agency business (personal and company vehicle) and transport clients as needed.
- Physical presence at the program is required.
- Bachelor's degree in a behavioral science, or a bachelor’s degree in related field that is directly related to the specific responsibilities of the job.
- Three (3) years' experience in the mental health field, with experience with the major diagnosis listed in the DSM 5, and the ability to identify the diagnosis and severity, or a master’s degree in a behavioral science related field (i.e., psychology, MSW, etc.).
- Demonstrated knowledge in mental health services with minors, adults, and/or families.
- Knowledge of mental illness and effective treatment approaches to mental disorders.
- Experience with co-occurring substance use disorders and treatment modalities.
- Ability to operate within the Electronic Medical Records (EMR) system.
- Ability to communicate effectively in written and spoken Spanish is preferred.
- Intermediate computer skills in Office 365, Teams, Zoom and other communication/meeting platforms.
- Possess a valid driver’s license and have access to a dependable means of transportation that is properly insured and operated in accordance with all laws. Agency insurer authorization is required.
- Ability to pass a pre-employment criminal background check.
- Ability to pass a pre-employment drug screen, and TB test.
