We are currently seeking to fill the MSWto service in FRESNO, MADERA, MARIPOSA and surrounding areas:

  • Acute care experience preferred
  • Compensation: hourly rate range: $40-$43 (negotiable: based on experience, skill level, availability, etc).
  • Salary option available, as well.
  • Opportunities to accumulate overtime.
Essential Job Functions/Responsibilities
  • Assesses the psychosocial status of patients related to the patient’s illness and environment and communicates findings to the registered nurse.
  • Carries out social evaluations and plans intervention based on evaluation findings.
  • Maintains clinical records on all patients referred to social work.
  • Prepares clinical and progress notes.
  • Provides information, referral services, and application assistance for community organizations and resources (Medi‑Cal, IHSS etc) for patients and families/caregivers. Provides information and links patients or families/caregivers with community agencies.
  • Serves as liaison between patients or families/caregivers and community agencies.
  • Maintains collaborative relationships with organization personnel to support patient care.
  • Maintains and develops contracts with public and private agencies as resources for patient and organization personnel.
  • Participates in the development of the total plan of care and case conferences as required.
  • Assists physicians and other team members in understanding significant social and emotional factors related to health problems.
  • Participates in discharge planning.
  • Other duties as delegated by the Clinical Director/Supervisor.

Position Qualifications
  • Graduate of a Master's program in social work accredited by the Council on Social Work Education. May also have a doctoral degree in social work.
  • Active ACSW registration with the California Board of Behavioral Sciences (BBS) required.
  • Minimum of one (1) year's experience in health care.
  • Experience in home health care preferred.
  • Demonstrates good verbal and written communication, and organization skills.
  • Possesses and maintains current CPR certification.
  • Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.
  • Requires proficiency in word processing, spreadsheets, and email, as well as basic computer operation.
  • Familiarity with how to complete resource linkage to local and state agencies.
  • Candidates must be eligible for and obtain Associate Clinical Social Worker (ACSW) registration within three (3) months of employment. Clinical supervision will be provided, and assistance with the ACSW application is available.
