This facility is seeking an Optometry Physician for locum tenens support as they look to fill a current need.

Details & requirements for this opportunity:

• Schedule:
  
  Monday – Friday, 8:00am-5:00pm.
• Practice Setting:
  Outpatient clinic.
• Types of Cases:
  Routine Eye Exams, Contact Lens Fittings, Eye Urgencies and Emergencies.
• Credentialing Timeframe: 45-60 days.
• Electronic Medical Record (EMR):
  Health Connect (Kaiser Permanente’s version of EPIC).
• Certifications
  
  Required:
  
  Cardiopulmonary Resuscitation (CPR) Card and Board Certification is required.
• Licensure
  
  Required:
  
  Active California License is required.
