The Challenge (Job Summary)
The Clinical Advocate’s primary responsibility is to assist with population management and development of advanced medical home model in primary care offices. The advocate will communicate policies and protocols and workflow changes to primary care offices. The advocate will assist with implementation of changes related to population management and support compliance with policy and procedure. This includes monitoring population management reports, pre-visit planning, and consistent office communication in the form of huddles and monthly office meetings.
As part of advanced medical home model, the Clinical Advocate will assist in health coaching and support implementation of patient care plans. The advocate will work under the direction of a supervising care manager/physician and serve as a liaison between a primary care office and the Care Management Team/administration.
Your Day To Day & Essential Functions
- Work as a liaison between the Care Management team and a primary care office to support adoption and compliance with the Advanced Medical Home model.
- Receive training and attend meetings with other clinical advocates to assist with uniform delivery of office policy and protocol.
- Support “best practice” policy and protocols are shared with office clinical staff and physicians.
- Aid with office workflow changes and use of protocols.
- Comply with workflow changes and use of protocols.
- Comply with documentation in appropriate EMR fields.
- Assist and provide oversight for population management reports, including outstanding lab, radiology, and referral reports; preventive services; recall processes, and chronic disease registries, under the direction of the supervising care coordinator.
- Assist and provide oversight in primary care offices in the adoption of population management tools, including organization of “best practices” educational tools and community resources for patient care.
- Participate in discussions of quality measures with office staff and providers.
- Receive training and assist in development of quality improvement plans
- Encourage use of daily huddles, and regular office meetings for communication within a practice.
- Assist at risk patients in following care plans, by providing health coaching for patients who need assistance in adopting healthy lifestyles. The advocate will be trained on use of motivational interviewing techniques to help patients adopt positive lifestyles.
- Maintain efficient and timely clinical documentation.
