- Under the supervision of the Program Coordinator, provide community‑based services to youth and adults with mental health and substance use disorders.
- Provide case management services including ongoing assessment of needs, skills to facilitate personal independence and daily living skills; coordinate the ITP, linkages with formal and natural community support systems; symptom monitoring; advocacy and outreach; interventions for symptom management; activities to report recovery, including integration with self‑help programs.
- Provide crisis support and stabilization activities: de‑escalate persons in crisis, assist therapists with hospitalizations when necessary, actively assist with safety planning, coordinate with treatment team and after‑hours services.
- Provide Day Treatment services to adults as assigned by the Program Coordinator.
- Scope and volume of duties and target populations will vary by site/department needs and will be assigned by the Program Coordinator.
- Render all services within department guidelines and in compliance with Medicaid/Medicare, HFAP, OhioMHAS, and CARF standards.
- Demonstrate a commitment to quality care through professional growth, best practices, and patient‑centered engagement to instill hope and meet patients where they are at in their readiness for change.
- Perform safety tasks: ongoing risk screening, documentation, safety plan development, crisis management coordination with the treatment team and supervisor, ensuring accurate and timely clinical documentation.
- Promote recovery through active and collaborative treatment planning. Foster independence by identifying and incorporating patients’ strengths, involving natural support persons, self‑help resources, and community integration activities as critical components of long‑term recovery.
- Provide assertive outreach for clients not engaging in treatment; extensive outreach efforts to at‑risk clients who may not be involved.
- Understand Patient Care Standards (PCS) as a measure of commitment to serve those in need; ensure PCS standards are met and respond to supervisory strategies to address challenges.
- Associate’s degree or higher in social services or related field (preferred). Experience with individuals with mental health or substance use problems (preferred).
- Heart Saver, Aegis Crisis Prevention, and First Aid certification within 6 months of hire.
- Effective written and oral communication skills, sensitivity to cultural needs.
- Punctual attendance at assigned work location required.
- Mobile, valid State of Ohio motor vehicle license, insurable under Firelands auto policy (provide proof of personal automobile liability insurance coverage upon request).
- Ability to work safely, maintain accident‑free workplace, knowledge of emergency codes.
- Compliance with OSHA regulations, CDC standard, transmission‑based precautions, and proper PPE. Ability to comply with applicable S.D.S. forms.
- Successful completion of a 180‑day probationary period.
