A Pro Medica RN Acute Care Navigator plays a key role in overseeing patient care, ensuring seamless care progression, and coordinating the appropriate level of care for their caseload. The focus includes optimizing length of stay, preventing readmissions, and ensuring compliance with value‑based programs.

The role involves conducting initial assessments, developing discharge plans, and collaborating with interdisciplinary teams. Patient education includes providing information on medications, treatments, and post‑discharge care. Throughout the patient’s stay, the Navigator monitors progress, reviews daily health records, and escalates any issues to leadership to ensure smooth transitions and positive patient outcomes.

• BSN or 5 years applicable nursing experience.
• Current, active, and unrestricted RN licensure in the state of practice.

• MSN or higher (desired).
• Experience in patient advocacy, care management, and knowledge of hospital and community resources.
• Case management or similar experience.
• Certification in care management (e.g., CCM RN‑BC, ACM, or similar).
