Anticipated Territory:
Alameda County (Hayward/Fremont).
Schedule: Saturday / Sunday.

• Conduct Start of Care (SOC) admissions – OASIS.
• Perform follow‑ups, evaluation visits in designated territory.
• Make home visits to patients.
• Reconcile medication, wound care, treatments, and procedures.
• Monitor clients’ conditions and report changes to the clinical team.
• Collaborate with physicians and agency staff to deliver the best care.
• Initiate and administer IV medication infusions safely according to protocols.
• Document patient care observations, interventions, and evaluations accurately.

• Concierge support for case managers to handle administrative tasks.
• Flexible schedules – create your own hours.
• Targeted territories—usually within a 5–15 mile radius.

• Current and unrestricted California RN license.
• Current BLS/CPR certification.
• Valid California driver’s license.
• Experience with assessments and SOC – OASIS.
• High level of clinical knowledge in home‑health care.
• Experience with high‑acuity patients.
• Experience working with interdisciplinary teams.
• Case management fundamentals knowledge.
• Experience with IV medication infusions.
• Proficiencies in initiating intravenous therapy.
