Home Health Physical Therapists (PT) provide personalized care to patients in their homes, focusing on improving mobility, managing pain, and enhancing overall physical function. They assess patients’ needs, design tailored treatment plans, and conduct therapies to promote recovery and well‑being within the comfort of home settings.

• Health Flex Assist:
  Dedicated in‑house resource for administrative duties to allow focus on patient care.
• Documentation support:
  Extensive services to minimize documentation burden, including quality assurance, transcription, and OASIS support.
• Medbridge:
  Access to an app for designing home exercise programs with 3D models explanatory videos, setting reminders, and tracking progress.
• Drive time minimized:
  Territory radius of 5‑10 miles, dependent upon location.
• Work/life balance:
  Flexible scheduling and autonomy managing your own hours.
• Structured training:
  Individualized, ongoing program to develop skills and provide industry‑leading patient care.
• Communication:
  Leading, easy‑to‑use documentation and technology solutions streamline communication.

• Current and unrestricted CA PT license.
• 1 year PT experience.
• Current AHA CPR certification.
• Valid driver’s license.
