Home Health Registered Nurse | Freeport, Illinois

Location:

Schedule:

• 8:00 AM to 5:00 PM, Monday through Friday
• Every third weekend required
• Rotating holidays

Responsibilities:

• Deliver professional nursing care in patients' homes according to established care plans
• Monitor patient conditions, administer IV therapy, and manage wound care as needed
• Collaborate with the healthcare team to ensure comprehensive patient care
• Educate patients and families on treatment plans, medications, and health maintenance
• Maintain accurate documentation and mileage records between visits

Qualifications:

• Current Illinois RN licensure and Basic Life Support (BLS) certification required
• Minimum of one year of nursing experience within the past three years in an acute care setting
• Experience preferred in home health, IV therapy, emergency room, or oncology settings
• Cardiac or wound care certification is a plus
