Intake Nurse; LPN - Hybrid

We are seeking a Full‑time Intake Nurse to work remotely and support our market in Freehold, NJ.

• Answer phone calls and receive all referral and intake information.
• Complete referral/intake forms and route them appropriately for admission approval.
• Assist in scheduling of appropriately skilled personnel and input scheduling information into the computer.
• Check common working files for Medicare coverage and verification of benefit period.
• Perform other duties as directed.

• Office setting that requires regular phone usage, sitting, stooping, and lifting of office supplies (e.g., printer paper stacks).
• The job requires the ability to utilize a PC, calculator, multi‑line telephone, and other related office equipment.

• High school graduate or GED.
• LPN license.
• Experience in a Home Health Care or Hospice setting preferred.
• Must be computer literate and able to maintain simple records.
• Must be able to communicate effectively orally and in writing.
• Must have at least 3 years of customer service experience.
• Must have at least 3 years of EMR System experience.

Pay starts at $62,500 salaried.
