Occupational Therapist - SNF

Contract Type: Travel Contract

Roles & Responsibilities- Provide patient-centered occupational therapy care in a skilled nursing facility
- Assess functional abilities and develop individualized treatment plans
- Deliver therapeutic interventions to improve ADLs, cognition, and mobility
- Collaborate with nurses, PT, SLP, and other team members to coordinate care
- Document progress accurately and timely to meet facility and regulatory standards
- Educate patients and families to support safe transitions and independence
- Participate in interdisciplinary meetings and contribute clinical insights
- Open to clinicians at all experience levels, including new graduates (posting explicitly welcomes new grads)
- Valid Occupational Therapist credential or ability to obtain state licensure (facility will confirm requirements)
- Experience or interest in working in a skilled nursing facility / rehab setting
- Strong communication and teamwork skills for interdisciplinary care
- Current CPR/BLS as required by facility (verify during onboarding)
- Graduate of an accredited Occupational Therapy program (entry-level qualifications expected; facility to confirm specific degree requirements)
- Skilled in ADL training, adaptive equipment recommendations, and therapeutic activities
- Competent with clinical documentation and care planning in a SNF environment
- Ability to evaluate functional mobility, cognition, and safety needs
- Strong clinical reasoning and patient education abilities
- Comfortable collaborating in a fast-paced, interdisciplinary setting
