We are seeking a dedicated and compassionate full-time Hospice Social Worker to join our team. In this role, you will provide psychosocial support to patients and their families, helping them navigate the emotional, social, and financial challenges associated with end-of-life care in the Freehold, NJ region. Your expertise will contribute to the development of comprehensive care plans and support our interdisciplinary team in delivering exceptional hospice care.
Responsibilities- Conduct a psychosocial assessment as part of the comprehensive assessment within 5 days of admission to the hospice program, identifying psychosocial problems, interventions, and goals for the patient’s plan of care.
- Participate in interdisciplinary group (IDG) meetings and the development of the patient’s plan of care.
- Observe, record and report changes in the patient’s emotional and social factors related to illness and care needs.
- Maintain and submit written clinical records, including initial evaluation, care plan and daily notes.
- Evaluate the patient and family response to, and effectiveness of, the social work intervention.
- Coordinate with Case Managers to schedule visits and facilitate coordination of other staff visits.
- Participate in IDG and QAPI meetings and activities.
- If the patient/family declines Social Work services, continue to offer support to the IDG and monitor evolving needs.
- Engage in ongoing professional development through education, workshops, conferences, and independent study.
- Assess the patient’s and family’s ability to cope with dying and provide appropriate support.
- Act as a consultant to health team members on social, emotional, and environmental factors related to health problems.
- Assist patients in utilizing family and community resources.
- Licensed by the NJ State Board of Social Work Examiners;
Master’s degree in social work from an accredited program. - Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW).
- At least one year of post-master’s social work experience in a healthcare setting.
- Hospice experience preferred.
- Ability to reliably handle and maintain confidential information.
- Must be a licensed driver with an insured automobile in good working order.
- Strong verbal and written communication skills.
Salary: $72,000
