Ennoble Care is seeking a full-time Clinical Care Coordinator, Embedded to join our team, with a primary focus on providing on-site, embedded support within one or more partnered long-term care facilities in the Fredericksburg, Virginia area. This role is an integral, in-person member of our clinical care team, working side-by-side with Ennoble Care providers in a family-practice style model-of-care that prioritizes continuity, relationship-building, and patient-centered support.

Key Responsibilities- Coordinate care for long-term care residents using Ennoble Care’s EMR
- Maintain regular, proactive, on-site engagement with residents to support care plans, follow-ups, and care transitions
- Collaborate closely with Ennoble Care providers within the long-term care setting
- Serve as a key liaison between providers, facility staff, residents, and families
- Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely manner
- Accurately document patient interactions and care coordination activities
- Follow established workflows, documentation standards, and escalation protocols
- Contribute positively to team collaboration and strong facility partnerships
This is not a remote or call-center role.
As an Embedded Care Coordinator, you are physically on-site in one or more partnered long-term care facilities, working alongside providers and facility staff. You build consistent relationships with residents and families and become a trusted, familiar presence within the facility, supporting care the way a traditional family practice would, but in the long-term care setting.
Qualifications- Legally authorized to work in the United States
- Active, unrestricted clinical certification or licensure, such as:
Medical Assistant (MA) / Certified Medical Assistant (CMA);
Licensed Practical Nurse (LPN);
Registered Nurse (RN) - Minimum 1 year of experience in a long-term care, skilled nursing, or similar clinical setting
- Comfortable communicating throughout the day via Microsoft Teams, email, phone, and in person
- Cultivate and maintain positive, approachable relationships with residents, families, providers, and facility staff, while demonstrating a consistent, dependable presence within the long-term care setting
- Strong organizational and time-management skills
- Working knowledge of healthcare terminology and care coordination principles
- Proficiency with Microsoft Outlook and Microsoft Office
- Ability to work on-site, full-time, Monday through Friday (approximately 8:30 a.m. to 5:00 p.m. EST)
- No on-call
- Daytime hours
- Seven (7) company-paid holidays annually
- Transportation criteria:
- Valid, unencumbered driver’s license
- Reliable personal vehicle
- Current auto insurance, with the ability to provide proof
- Please note, travel between facilities may be required
- Ability to pass a background check and drug screen
