The Physical Therapist (PT) provides high-quality, patient-centered care, managing therapeutic care for assigned patients in coordination with physicians and the care team. Responsibilities include patient assessments, developing care plans, and supporting patients throughout their care continuum.

• Conduct evaluations and interventions within legal and ethical practice.
• Assess therapeutic/functional status and develop care plans.
• Provide therapeutic care at patients' homes using appropriate techniques.
• Educate patients and caregivers on therapies and devices.
• Prioritize caseloads and coordinate staffing and handoffs.

• Bachelor's or higher in Physical Therapy.
• Preferred experience in Acute or Outpatient Physical Therapy.
• Valid Physical Therapy License from Texas State.
• Basic Cardiac Life Support certification.
