The Physical Therapist (PT) provides high-quality, patient-centered care, managing therapeutic care for assigned patients in coordination with physicians and the care team. Responsibilities include patient assessments, developing care plans, and supporting patients throughout their care continuum.
Key Responsibilities:- Conduct evaluations and interventions within legal and ethical practice.
- Assess therapeutic/functional status and develop care plans.
- Provide therapeutic care at patients' homes using appropriate techniques.
- Educate patients and caregivers on therapies and devices.
- Prioritize caseloads and coordinate staffing and handoffs.
- Bachelor's or higher in Physical Therapy.
- Preferred experience in Acute or Outpatient Physical Therapy.
- Valid Physical Therapy License from Texas State.
- Basic Cardiac Life Support certification.
