Case Manager (Service Coordinator)
Service Coordination Inc is seeking a passionate Service Coordinator to provide case management services to individuals with intellectual and developmental disabilities. The role requires building strong relationships with clients, families, and community partners to develop and implement person‑centered plans that meet state‑mandated standards.

Provide case management services, establish positive working relationships, and develop person‑centered plans. Meet state‑mandated requirements, secure ongoing support services, and ensure all services meet quality and compliance standards.
Essential Duties- Advocate on behalf of individuals and provide resource information.
- Educate individuals, families, and support staff on self‑determination and service coordination.
- Collect eligibility documentation for DDA services and recommend priority to DDA.
- Conduct comprehensive assessments, develop and update Person‑Centered Plans (PCPs), and write Self‑Directed Plans.
- Research, refer, and coordinate resources to achieve PCP outcomes.
- Travel to meet with support staff and conduct on‑site visits.
- Assist with transitions, paperwork, wafer follow‑ups, and documentation for medical assistance.
- Submit required recertification and investigation reports; apply for new programs and services.
- Communicate with providers to resolve client concerns and ensure compliance with SCI policies.
- Meet performance utilization benchmarks and maintain secure HIPAA records.
- Utilize tracking and time‑management tools.

- Bachelor’s degree in a human services field.
- Or bachelor’s in a non‑human services field with two years of experience in a human services field.
Experience working with people with developmental disabilities.
Physical DemandsStandard office environment; maintain a stationary position >95% of the time. Operate computer equipment and occasionally move office equipment weighing 10–25 pounds. Must have visual acuity for close work and the ability to operate a motor vehicle.
