Seeking a Hospice RN to work Part-time, 64 hours bi-weekly, Monday - Thursday night shifts from 12am - 8:30am. This RN is responsible for triaging after-hours calls and responding with a skilled nurse visit as appropriate. Will provide skilled professional nursing care to the patient as prescribed by the physician and in compliance with regulations as established by the State Board of Nursing and department policies.
The RN/Case Manager coordinates the interdisciplinary care team for the patient and family to assure continuity of care. Services may be provided to neonate, infant, child, adolescent, adult and geriatric populations.

Essential Functions:
- Completes assessment and admission of patients as assigned.
- Assesses and regularly re-evaluates patient and family needs.
- Performs basic physical assessment of all body systems.
- Assess physical environment including safety factors
- Revises plan of care as necessary.
- Documents assessment and completes all required forms including consents in compliance with policies and procedures.
- Utilization of all elements of the nursing process.
- Promotes optimum patient comfort as part of an interdisciplinary team under the attending physician’s orders.
- Provides case management for assigned patients.
- Initiates and sustains implementation of orders as prescribed by the physician.
- Notifies attending physician of any changes in care plan.
- Is compliant at all times with policies regarding documentation.
- Monitors documentation in the clinical record and initiates the discharge planning process for assigned patients.
- Assures that all team members have completed the discharge summary.
- Uses supplies and equipment effectively and efficiently, meeting applicable competency standards.
- Provides medication, diet and other instructions as ordered by the physician.
- Recognizes and utilizes opportunities for health counseling.
- Works in concert with the other interdisciplinary team members.

- Carries out basic Infection Control procedures.
- Disposes of dressings and other waste according to protocol.
- Recognizes and validates the psycho/ social/ spiritual state of individuals in the home.
- Makes a supervisory visit to observe the work of Home Health Aides at least every 14 days.
- Keeps appropriate team members apprised of changes and updates to patient/family/caregiver status.
- Maintains and upgrades professional knowledge and practice skills.
- Participates in continuing education and in-service classes.
- Stays informed of department/hospital events by reading minutes or attending staff meetings 100% of time.
- Actively participates in quality management programs.
- Interacts in a calm, courteous and professional manner.
Required
Knowledge, Skills and Abilities:
- Demonstrates clinical competency in area of practice. Understands Hospice philosophy, principles of death and dying. Knowledgeable about Hospice eligibility requirements.
- Has excellent observation, assessment, organizational and communication skills, and good nursing judgment. Ability to provide good written documentation in a timely manner.
- Self-directed with the ability to work with little supervision.
- Flexible and cooperative in fulfilling role obligation and has ability to work effectively within an interdisciplinary team.
- Demonstrates ability to assess and respond to the needs of patients and families. Interacts in a calm, courteous and professional manner.
Minimum Education , Training, and Experience
Required:
- Currently licensed as a Registered Nurse in the State of Maryland.
- Possesses and maintains current CPR certification.
- At least 2 years general nursing experience is required with current Hospice, medical, surgical, critical care or home health experience. Experience providing pain management, IV skills preferred. Hospice nursing experience preferred.
- Bachelors of Science in Nursing Required or Associates degree in Nursing with a signed
- BSN agreement to obtain BSN within 5 years of employment.
